Just like other types of doctors, we were taught at chiropractic school that not much can be done for peripheral neuropathy. We learned that the nerve damage in your hands and feet due to things like diabetes, chemotherapy, or alcoholism were permanent and progressive. However, in recent years doctors started to notice something interesting when they were using class 4 laser therapy to treat other conditions. If they were treating someone for plantar fasciitis, for example, they found that in many cases the patient’s neuropathy symptoms would also improve! At King Chiropractic & Laser, we had already been using class 4 laser therapy since 2015 to treat all kinds of musculoskeletal conditions. In 2022, we decided to upgrade to a higher-powered laser. When the rep from Summus Medical Laser started telling us about using the laser to treat neuropathy, we were understandably skeptical at first. However, after reading the research and talking to other doctors around the country who had been using Summus’s neuropathy protocol with great success, we decided to take the leap!

How are you treating neuropathy?

Primarily we are using Class 4 laser therapy, which uses 4 wavelengths of red and infrared light that have been shown to cause beneficial photochemical reactions in the cells. We also incorporate supplements and exercises as needed to improve outcomes. 

How does it work?

Laser therapy, aka photobiomodulation, works by penetrating deep into the tissues and causing several things to happen:

 

  1. Laser is absorbed by the mitochondria in the cells, which boosts ATP production. ATP is the fuel of the cells, so this effectively gives your cells a little energy boosts.
  2. Laser decreases inflammatory chemicals like cytokines, thus reducing swelling and pain.
  3. Laser causes the release of nitric oxide, which dilates the blood vessels and causes increased blood circulation to the treatment area.
  4. Over several treatments, laser also stimulates the growth of new blood vessels into the area.
  5. Laser stimulates the release of endorphins, your body’s natural pain reliever.
Is laser treatment approved by the FDA?

Yes, Class 4 laser was approved by the FDA in 2003 for medical use. 

Is it painful?

Not at all. In fact, most people find it very soothing. It feels like a gentle warm massage. 

Are there any side effects?

Typically there are none. Occasionally people will have a temporary flare up of pain or redness at the treatment site, but this is not common.

How soon will I feel results?

Usually people start to notice less pain and more sensation after 3-4 treatments, although we have had some that feel a difference even after one treatment!

Will I have to keep doing it forever?

Typically after 16 treatments we are able to get at least 70% reduction in neuropathy symptoms. On the 16th visit, we do a re-evaluation and compare the exam results to the initial visit (pin prick test, vibration, muscle strength). If you are still seeing incremental improvements, we will recommend doing a few more treatments. If you are either 100% better or your progress has plateaued, we discontinue treatment. Most people do not see any return in symptoms. About 10-15% of people do have some regression over time, usually due to poor blood sugar control or advanced age. In these cases we suggest coming in for an occasional “booster” treatment. We have been tracking our patients since we started neuropathy treatments in 2022 and most of them are still feeling great!

Why have I never heard of this treatment?

Doctors have been using laser therapy for decades for musculoskeletal issues, but using it for neuropathy is a more recent development. There have been some very promising studies, but we definitely need more before it is widely adopted. Your doctor hasn’t been keeping it from you, they just probably don’t know about it yet! 

Why is this not covered by insurance?

It takes a long time, lots of studies, and pressure from lobbyists to get insurance companies to cover anything. Laser therapy for neuropathy is a relatively newer treatment. We’ve had some very promising studies but we need a lot more before it is widely accepted by traditional medicine. We also don’t have any big pharma companies lobbying for it to be covered!

How much does it cost?

It is only $50 to come in for a consultation and exam with Dr. King. After going over your medical history and doing some neurological testing, she let you know what treatment plan would work best for you.  There are many different types of neuropathy and they are all treated differently, so it can be difficult to give an exact estimate before seeing someone. For a typical diabetic neuropathy patient, we do a package for $2000 that includes 16 laser treatments, supplements (if needed), exercises, use of our vibration plate, magnesium or CBD lotion application after each treatment, and a re-evaluation with Dr. King at the 16th visit. After you complete the 16 treatments, if you need any “booster” treatments, the price is $100 per treatment.

Do you offer payment plans?

We want people to be able to get the treatment they need, so we are willing to work with people to get it to fit into their budget. We also accept Care Credit, which allows you to spread out payments over 6-24 months.

What is peripheral neuropathy?

Peripheral neuropathy refers to damage to the little nerves in the extremities. It can be caused by many things:

  • Diabetes
  • Pre-diabetes or borderline diabetes
  • Medications
  • Chemotherapy
  • B12 deficiency
  • Alcohol abuse
  • Infections
  • Poor circulation
  • Old age

There are also other conditions that can mimic peripheral neuropathy, like sciatica, “pinched” nerves in the spine, brain or spinal cord damage, multiple sclerosis, and Morton’s Neuroma. Since some of these things are unresponsive to laser therapy, Dr. King always likes to do a quick phone consultation before scheduling a neuropathy exam.

Peripheral neuropathy presents a little differently in every patient, so not everyone is going to have all of these symptoms, but if you have a few of these symptoms, you most likely have some form of peripheral neuropathy:

  • Numbness or tingling in both feet (or hands)
  • Cold hands or feet
  • Stinging or zapping pains in the feet (or hands)
  • Burning sensation or hypersensitivity- some people say even the bedsheets touching their feet is painful
  • Stiffness in the feet or toes
  • Discoloration of the skin- sometimes a purplish tinge in the toes, sometimes a brownish discoloration in the legs and feet (poor circulation)
  • Dry, flaky skin and yellowed nails (also poor circulation)
  • Poor balance
  • Symptoms are usually pretty symmetrical between the left and right sides
  • Symptoms typically start furthest from the center of the body and work inward (for example, you might start to notice numbness in the toes that gradually spreads up the feet into the legs)

Over 20 million people suffer from peripheral neuropathy in the U.S. alone. If left untreated, neuropathy can become very debilitating and can lead to:

  • Inability to walk
  • Inability to drive
  • Increased risk of falling and fractures
  • Infections and deformities of feet
  • Amputations

Traditional medical treatments for neuropathy focus on managing the symptoms by using medications. These medications do not fix the underlying cause of the neuropathy, and they can also cause annoying side effects and risk of dependency.

How Does Diabetes Cause Neuropathy?

 

Diabetes is by far the most common cause of neuropathy. More than half of diabetics will eventually experience some degree of neuropathy. Even people with “pre-diabetes” can start to develop neuropathy. But how does high blood sugar damage the nerves?

Having high blood sugar damages your blood vessels in a few different ways. The excess sugar itself is very inflammatory. It is like having little shards of glass floating around in the blood vessels, damaging the inner lining. Most people with diabetes also have high cholesterol and triglycerides. Inflamed blood vessels tend to trap cholesterol and form plaques, or “atherosclerosis”. This makes the blood vessels harder and narrower. When the arteries are affected, this can lead to heart attacks, strokes, and kidney failure. When the tiny capillaries are affected, it can lead to nerve damage and blindness.

The tiny nerves at the ends of the hands and feet tend to be affected first with diabetic neuropathy. When capillaries that feed nerves are damaged, the nerves no longer get the nutrients and oxygen that they need. Essentially, they are starved. Over time, they stop functioning normally and you begin to feel numbness, tingling, and pain. This is a progressive problem and will get worse over time if left untreated.

Most of the time, the tiny nerves are just damaged and can be revived if proper circulation is restored. That is why it is so important to identify neuropathy as soon as possible and to treat the root cause of the problem- poor circulation. Medications may ease the symptoms of neuropathy, but to truly fix the problem and prevent life-long damage you need to ensure proper blood flow. Laser therapy is the best way to accomplish this.
